Obviously I have no experience of this exact journey, just wanted to say don't be afraid of the London Underground. I have a very bad sense of direction and am not especially familiar with the underground there, but have always found it easy to navigate. Everything is clearly signposted and there are maps everywhere. The walk is nicer from Baron's Court, through the cemetery instead of down the main road - however, it does take a bit longer. I'd definitely recommend going via Hammersmith if you're not sure of where you're going though, it's much easier.
IM's directions sound pretty good to me - her memory is obviously better than mine, I couldn't have described it nearly so well!
There's a Starbucks just past the clinic on the other side of the main road if you need somewhere for a coffee and sit down before your appointment.

btw, as well as checking the tfl website for service closures, it is well worth asking the station staff too. Sometimes the website doesn't get updated with line closure information, but the train station staff always seem to know. _________________ The boobs are just a set of metaphors.

i agree with flameboy. barons court is a nicer station, there is a lovely coffee shop on the left as you come out of it (they do good cake), and the walk through margavine cemetry is a nice way to clear your head pre-appointment. i don't know if it's further, but the route is a little more complicated. maybe it's one to try and figure out on the way back when you're less stressed.
seriously, the coffee shop is sweet.
edit: just read properly! ha. you've already been. ne'er mind
